This test is used to identify patients with either primary carnitine deficiency, as well as patients with secondary carnitine deficiency arising from other inborn errors of metabolism or renal dysfunction. Free (non-esterified) and total carnitine are determined by tandem mass spectrometry following stable isotope dilution, and the acyl- (esterified) fraction is calculated by subtraction.
