The D-dimer test utilizes a monoclonal antibody that recognizes a cross-linked fibrin epitope. The qualitative D-dimer assay is used to assess the presence of disseminated intravascular coagulation (DIC), a condition characterized by unregulated activation of both thrombin and plasmin. D-dimer may be elevated in patients with venous thrombosis or pulmonary embolism, reflecting physiologic fibrinolysis. Patients with rheumatoid factor may have false-positive results. D-dimer may be elevated in patients with phlebitis and following surgery reflecting physiologic fibrinolysis. |
